Video Terms and Conditions

By checking the acknowledgement box on The Right Time website, I acknowledge that I appear in and as part of the video
footage in which I describe my experience with The Right Time. (the “Photo/Video”).

In full and complete consideration of your possibly including the Video in a public service campaign for The Right Time
conducted by Power to Decide (“Power to Decide”), and without any further consideration due from Power to Decide or
any third party to me, and with the understanding that Power to Decide, in its sole discretion may choose whether or not
to include the Video in Power to Decide campagins, websites, or other collateral, I hereby agree as follows:

I hereby grant to the Power to Decide and to other such persons as Power to Decide may designate from time to time, the
absolute right and permission to use my image, likeness, persona, photograph, picture, voice, statements, biographical
information, name and/or any other information, as embodied in the photo or video, in whole or in part, or distorted in
character or form, either alone or accompanied by other material, throughout the United States its territories and
possessions in perpetuity, in any manner, for the purpose of advertising, publicity, trade or any other lawful purpose
whatsoever, in any media now known or hereafter developed, including, but not limited to, the Internet. Said right and
permission shall be worldwide as it relates to digital media and Internet usage. I hereby waive all rights to inspect and
approve the finished product, its use, or such written or spoken copy as may be used in connection therewith, in connection
with Power to Decide. You shall also have the unrestricted right to edit the content and text of the Photo/Video in any
manner or form. The Photo/Video or any part thereof (including without limitation my likeness and/or performance) can
be edited, cut, added to, adapted, dubbed, modified and/or rearranged at Power to Decide’s sole and absolute discretion. I
acknowledge that the use of my likeness and/or performance in the Photo/Video after alteration by Power to Decide may
be used in connection with a campaign to prevent teen and unplanned pregnancy. I hereby waive unconditionally and
irrevocably the benefit of any provision of law known as "moral rights" or similar laws now or hereafter prevailing in any
part of the world.

I agree that I will not hold Power to Decide, Client, or anyone who receives permission from either of them, responsible
for any liability resulting from the use of the Video in accordance with the terms hereof, including what might be
deemed to be misrepresentation due to distortion, optical illusion or faulty reproduction which may occur in the finished
product or for any claims based on right of privacy, right of publicity, defamation and/or false light.

I am over 18 years of age and have the right to make this agreement.
